Prostate cancer, a prevalent malignancy among men globally, often necessitates androgen deprivation therapy (ADT) as a vital treatment modality. Although ADT effectively inhibits tumor growth, it also disrupts bone remodeling, leading to bone loss and an elevated risk of osteoporosis in patients. Osteoporosis, characterized by reduced bone mineral density and structural deterioration, poses significant health risks in individuals on long-term ADT.
Bone mineral density (BMD) screening is essential for identifying osteoporosis and assessing fracture risk in prostate cancer patients on ADT. However, clinical practice often overlooks this critical aspect, resulting in alarmingly low rates of BMD screening. Studies have shown that only a small fraction of patients undergo dual-energy x-ray absorptiometry (DXA) testing, highlighting the urgent need to enhance awareness and proactive management of bone health in this population.
To address the inadequacy in BMD screening, a pioneering initiative introduced an automated outpatient system with a pop-up reminder to prompt physicians to arrange bone health screenings for patients prescribed ADT without recent DXA evaluations. The system significantly increased screening rates, with a substantial improvement observed in patient awareness and adherence to calcium supplements and bone protective agents post-screening.
By retrospectively analyzing BMD screening rates from 2000 to 2018 and comparing them to the period after implementing the auto-recruit system, a remarkable surge in screening compliance was evident. Participants utilizing ADT for over a year exhibited poorer T-scores and higher osteoporosis rates, underscoring the necessity for timely and regular BMD assessments in this cohort.
Factors influencing patient participation in screening, such as age, duration of ADT use, and usage of bone-modifying agents, were analyzed to gauge the effectiveness of the screening protocol. The study revealed a significant increase in calcium supplement and bone health agent utilization post-screening, emphasizing the positive impact of the auto-recruit system on patient engagement and treatment adherence.
While the study demonstrated a notable uptick in BMD screening rates, challenges persist in achieving optimal screening compliance. Factors such as patient eligibility criteria, physician discretion, and existing treatments may influence screening decisions, necessitating a comprehensive approach to address barriers and enhance bone health management in prostate cancer patients on ADT.
Overall, the study underscores the critical importance of regular BMD screening in prostate cancer patients undergoing ADT and the role of innovative systems in improving surveillance rates and patient outcomes. By streamlining the screening process and enhancing patient education, healthcare providers can effectively identify high-risk individuals, implement preventive measures, and mitigate the adverse effects of bone loss associated with ADT.
